Position Summary

The Health Information Manager (HIM) works closely with the COO to oversee the interaction between and among the EMR and the data systems that collect and report information required for the care and oversight of RHC’s patient population. This work requires interaction between and among Clinical Providers, Medical Records, Care Management and Referral Coordination staff. The HIM also interact with payors to review gaps in care and facilitate the best possible outcome for risk-based contracts. They will collaborate with the Providers and review care needs and validate these needs against the medical record. They support quality outcomes and high reliability in the Patient Centered Medical Home. This role also acts as the HIPAA Privacy Officer

Scope Tasks & Responsibilities of Health Information Manager:

· Generate reports and extract data from databases using eClinical Works, Azara, Aledade, and Practice Analytics

· Develop and implement necessary communication strategy for improvement and awareness of quality issues for RHC

· Perform regularly scheduled production processes as well as one-time project specific request including the annual quality and clinical components of the UDS report

· Oversee IHIE reports and provide the data to the appropriate RHC personnel

· Advise RHC administration on the organization and reporting data in the organization as requested by supervisor

· Monitor performance according to agreed quality standards and take necessary action to communicate/advise/assist according to performance levels as requested by supervisor

· Resolve errors and omissions and addressing process shortcomings identified by such data quality reports and audits

· Act as point person with customers and suppliers where impacting/affected by quality issues as requested by supervisor

· Process Gaps in Care reports from MCE’s

· Provide data or other information for routine and committee reports for presentations, dashboards, and operational improvement activities as requested

· Lead team members to complete MCE quality activities for quality bonuses based on these measures

· Work with Risk Manager to update policies and procedures to reflect updated workflows

· Commitment to and ensure work is operating in a manner consistent to the Raphael Health Center mission

· Assist with health data implementations as needed

· Serve as Electronic Medical Record super user and subject matter expert.

Under HIPAA (the Health Insurance Portability and Accountability Act of 1996) every healthcare organization must designate a privacy official. The privacy official may have other titles and duties in addition to his/her privacy official designation in a typical practice or organizational setting. In terms of HIPAA compliance, the privacy official shall oversee all ongoing activities related to the development, implementation and maintenance f the practice/organization ’s privacy policies in accordance with applicable federal and state laws. HIPAA for purposes of this document includes HIPAA, HITECH and Omnibus requirements.

Scope Tasks & Responsibilities of Privacy Officer:

· Builds a strategic and comprehensive privacy program that defines, develops and maintains and implements policies and processes that enable consistent, effective privacy practices which minimize risk and ensure the confidentiality of protected health information (PHI), paper and /or electronic, across all medical types. Ensures privacy forms, policies, standards and procedures are up-to-date.

· Works with organization’s senior management and security to establish governance for the privacy program.

· Collaborate with information Technology (IT) to ensure alignment between security and privacy compliance programs including polices, practices, investigations and acts as a liaison to the information systems department

· Establishes with the information Technology an ongoing process to track, investigate and report inappropriate access and disclosure of protected health information. Monitor patters of inappropriate access and/ or disclosure of protected health information

· Performs or oversees initial and periodic information privacy risk assessment/analysis, mitigation and remediation.

· Conduct related ongoing compliance monitoring activities in coordination with the organization’s other compliance and operational assessment functions.

· Takes a lead role to ensures the organization has and maintains appropriate privacy and confidentiality consents, authorization forms and information notices and material reflecting current organization and legal practices and requirements

· Oversees, develops and delivers initial and ongoing privacy training to the workforce

· Participate in the development, implementation and ongoing compliance monitoring of all business associates and business associate agreements, to ensure all privacy concerns, requirements and responsibilities are addressed

· Works cooperatively with IT and other applicable organization units in overseeing patient rights it inspects, amend and restrict access to protected health information when appropriate

· Managers all required breach determination and notification processes under HIPAA and applicable State breach rules and requirements

· Establishes and administers a process for investigating and acting on privacy and security complaints

· Performs required breach risk assessment, documentation and mitigation. Works with Human Resources to ensure consistent application of sanctions for privacy violations

· Maintains current knowledge of applicable federal and state privacy laws and accreditation standards

· Cooperates wit the U.S. Department of Health and Huan Services Office for Civil Rights, State regulator and/or other legal entities in any compliance reviews or investigations

· Serves as information privacy resource to the organization regarding release of information and to all departments for all privacy related issues

Requirements:

Required Education, Certification, Experience and Skill:

· Bachelor degree preferred with health information management and/or Nursing or related healthcare field.

· Demonstrated healthcare reporting and database experience

· Demonstrated knowledge of EMR functions, design, strengths and weaknesses

· Strong demonstrated Microsoft Office experience

· Possess excellent analytical skills, understanding of reporting out of relational databases; keen interest in technology and a strong knowledge of eClincial Works (eCW), Azara, Aledade, etc.

· Strong communication skills with customer driven focus

· Ability to manage and complete multiple projects

· Capable of working with minimal supervision
