Description:Assertive Community Treatment (ACT) is a client centered, recovery oriented mental health service delivery model. The ACT team Psychiatric Nurse is a member of the ACT multidisciplinary team and is responsible for the following: Providing management and administration of medication in conjunction with the psychiatrist. The ACT Team nurse is also responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, providing a range of treatments including psychosocial rehabilitation. Will also provide support services to the ACT Team. This is a position with a lot of variety, community interaction, fast-paced, team oriented, with caseloads designed for optimization of client care.
This position reports to the Nursing Services Manager and is partially community based.
