Overview:
We are seeking a compassionate, highly skilled Nurse Practitioner (NP) to join our dynamic healthcare team, providing high-quality care to an adult patient population in a primary care setting. This is a hybrid role offering flexibility with 4 days working from home and 1 day in the office per week. You'll play a vital role in managing patient health, providing evidence-based care, and ensuring seamless coordination of services.

Key Responsibilities:
Provide comprehensive care to adult patients, including wound management, health assessments, and ongoing medical management.
Review lab and X-ray results, sick call requests, and coordinate follow-up care such as PPD and CXR results.
Oversee medication management and provide discharge planning to ensure continuity of care.
Address common primary care issues such as diet-related concerns, chronic conditions, and preventative care.
Collaborate with interdisciplinary teams to create effective care plans and enhance patient outcomes.
Ensure timely and accurate documentation of patient interactions and care provided.
Qualifications:
Board Certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Acute Care Nurse Practitioner (ACNP).
Previous experience in an emergency room setting is a plus, but not required.
Strong clinical background with a solid understanding of wound management, medication management, and the ability to review and interpret diagnostic data (labs, X-rays, etc.).
Excellent communication and patient care coordination skills.
Ability to work independently and as part of a collaborative healthcare team.
