The House Calls Medical Assistant/Scribe (CIS/MA) fulfills a crucial role for our in-home care teams; supporting logistics, clinical care, and documentation in our innovative home-based care program. The CIS/MA works directly with a House Calls Provider on a daily basis to help prepare patient visits, document visits in the EMR accurately, identify and bridge gaps in care, and advocate for patients and families when patients return from the hospital or experience other challenges. Responsibilities include preparing daily visit schedules, accompanying House Calls Provider on home visits, checking vital signs, administering vaccinations, performing point of care tests, blood draws for lab testing, assisting in reconciling patient medications, supporting patient and family education, documenting patient encounters in a structured note including history of present illness, assessment, plan, and physical exam, assigning appropriate CPT and ICD-10 codes, preparing After Visit Summaries, consulting with provider to ensure accurate documentation, improving clinical documentation, requesting and reviewing medical records, leveraging population health tools to support clinical documentation improvement in a value-based care model, preparing for and supporting daily huddles and clinical documentation reviews, providing administrative support for provider and care team, processing and managing orders and referrals, maintaining supplies and equipment, participating in daily huddles and weekly interdisciplinary team meetings, and other duties as assigned. Required qualifications include state or national Medical Assistant certification, CPR or BLS Certification, minimum 1 year experience as a Medical Assistant, passing all clinical competencies before supporting patient care, daily access to a vehicle with valid license and insurance, flexibility to travel throughout service area, electronic medical record experience, computer skills, bilingual (English and Spanish) a plus, and US work authorization.
