Housing Navigator - CalAIM Program
Location: [Merced County, California]
Full-Time | Mon-Fri (8a-3p)
We're looking for a Housing Navigator - Hybrid to help individuals and families experiencing homelessness or housing instability. You'll guide members through housing navigation, connect them with supportive services, and help them secure and maintain stable housing. You'll work closely with community partners, landlords, and care teams to make a real impact.
What You'll DoConduct housing assessments and create individualized housing plans.
Help members find affordable housing, apply for subsidies, and move in safely.
Connect members to healthcare, education, employment, and other supportive services.
Build relationships with landlords, property managers, and community partners.
Provide guidance on tenant rights and help resolve housing challenges.
Maintain accurate documentation in HMIS, Salesforce, and other systems.
Support members in preventing eviction and sustaining housing long-term.
Required:
3+ years in social services, case management, or housing support.
Knowledge of housing programs, HUD regulations, and community resources.
Strong communication, organization, and problem-solving skills.
Valid California driver's license & car insurance.
Preferred:
Experience with Coordinated Entry Systems (CES) and local housing authorities.
Training in Motivational Interviewing, Trauma-Informed Care, or Harm Reduction.
Experience building partnerships with landlords or property managers.
