We are currently seeking a Health Coach to join our exceptional Centralized Clinical Services team. As a Health Coach, you will play a vital role in supporting diverse patient populations by providing health and wellness coaching. You will collaborate with individuals to co-create personalized action plans aimed at achieving their unique health goals. This position involves facilitating coaching sessions both individually and in groups, which may be conducted virtually or in-person where available. This role is integral to our care team and offers a rewarding opportunity to make a positive impact on individuals' lives. Responsibilities include conducting personalized one-on-one coaching sessions focused on individual health goals using a member-centered approach, completing thorough documentation of visits, understanding members' health conditions and trends, conducting outreach calls, performing screening tools and measurements, scheduling and coordinating appointments, facilitating group coaching sessions, demonstrating empathy and ownership in resolving member concerns, connecting members with digital health platforms and community resources, supporting the care team, adhering to NBHWC Code of Ethics and health coaching scope of practice, participating in wellness campaigns, ensuring adherence to company policies including PHI/PII protection, escalating clinical or technical concerns, embracing routine feedback, and performing other duties as assigned. Qualifications include minimum 1-3 years of health coaching experience preferably in healthcare, bachelor's degree in health and wellness preferred, NBC-HWC certification preferred or required within 12 months, strong customer service skills, understanding of coaching vs health education and behavioral health, telehealth experience preferred, familiarity with EMR systems, Salesforce, Google Suite, exceptional communication skills, strong organizational skills, flexibility in work hours, bilingual skills a plus. Employee health requirements include documentation of Hep B vaccination or immunity, negative TB test within last 12 months, CPR certification (BLS), annual influenza and COVID-19 vaccinations. Physical demands include desk work, phone communication, possible travel to client onsite events, manual dexterity, and normal corrected vision, hearing, and speech.
