Job Summary
The Clinical Pharmacist assists the Division Director in providing specialized pharmaceutical services to include advising physicians on matters pertaining to drug usage and control within assigned hospital locations; and participate in the development and delivery of continuing education programs for Pharmacy, Patient Care Services, Medical staff and students.
Essential Duties and Responsibilities
• In conjunction with the Division Director, develop clinical pharmacy programs according to policies and regulations.
• Reviews prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Provides information and advice regarding drug interactions, side effects, dosage, and proper medication storage.
• Assesses the identity, strength, or purity of medications. Compounds and dispense medications as prescribed by doctors and dentists by calculating, weighing, measuring, and mixing ingredients, or overseeing these activities.
• Maintains records such as pharmacy files, patient profiles, charge system files, inventories, control records for radioactive nuclei, and registries of poisons, narcotics, and controlled drugs.
• Plans, implements, and maintains procedures for mixing, packaging, and labeling pharmaceuticals according to policy and legal requirements to ensure quality, security, and proper disposal.
• Determine and resolve medication therapy problems including but not limited to therapy inappropriate for medical conditions; immunizations; inappropriate doses, inappropriate dosage forms; medication allergies; inappropriate duration, schedule, route of administration, method of administration and therapeutic duplications.
• Monitor and intervene for clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.
• Verify medication orders for appropriateness and accuracy.
• Provide concise, accurate, and timely responses for pharmacokinetic drug dose calculations and requests for drug information.
• Participates in formulary management.
• Assists in medication reconciliation.
• Evaluates, documents, and reports medication errors and adverse drug reactions.
• Interprets, processes, and fills physician orders and prescriptions.
• Performs or oversees intravenous fluid compounding and manipulation using aseptic technique.
• Maintains knowledge of and seeks opportunities to improve accurate, timely, effective, and efficient medication delivery systems.
• Collaborates with other health care professionals to plan, monitor, review, and evaluate the quality and effectiveness of drugs and drug regimens, providing advice on drug applications and characteristics. Analyzes prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.
• Supervises and assists support personnel to complete daily tasks and responsibilities including unit does fills, prescription preparation, floor stock maintenance, and intravenous solution fills.
• Orders and purchases pharmaceutical supplies, medical supplies, and drugs, maintaining stock and storing and handling it properly.
• Assists in preparation of policies and procedures as requested.
• Participates in and helps implement the Quality Assurance and Performance Improvement (QAPI) for the assigned departments.
• Practices and behaves in a professional, safe and ethical manner at all times.
• Maintains friendly and courteous interpersonal communication skills and a positive service attitude.
• Participates in performance improvement and educational activities. Completes all CEU’s as required by respective licensure. If licensed, maintains an active and current license.
• Works willingly and accepts assignments as given.
• Attends all mandatory meetings and programs.
• Participates in opportunities that enhance personal and professional growth.
• Performs other duties as necessary or assigned.
• Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.
• Complies with all applicable state and federal regulations and JCAHO standards.
• Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.
• Maintains the confidentiality of patient, resident, employee and organizational information.
Supervision Responsibilities
This job has no supervisory responsibilities, however, may assist and provide training, education, and guidance to assigned staff.
Carries out responsibilities in accordance with the department Director, organization’s policies and applicable laws.
Qualifications

Education and/or Experience
• Bachelor’s degree in pharmacy or Doctor of Pharmacy degree.
• Proven experience as a clinical pharmacist.
Position Requirements, Certificates, Licenses, Registrations
• Currently licensed to practice pharmacy in the State of Iowa.
• Strong patient service skills.
• Excellent understanding of pharmaceutical therapy and direct patient care.
• Strong knowledge of medical and biological terminology.
• Excellent attention to detail and ability to balance competing priorities.
Language Skills
Ability to read, analyze and interpret technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from other healthcare providers, patients, families, regulatory agencies, or members of the community. Ability to effectively present information to management, the public, and/or board of directors.
