About the Role

We are seeking a compassionate and dedicated Clinical Case Manager to support the HUD-VASH Program, providing case management and supportive services to veterans experiencing homelessness. This role plays a key part in helping individuals reintegrate into the community by addressing their unique needs and barriers to stability.

What You'll Do

Provide mobile and field-based case management to support independent living skills, housing stabilization, employment linkage, benefits establishment, and community reintegration.

Work collaboratively with veterans, VA partners, and community providers to create and maintain Housing Stability Plans (HSP) that align with each individual's goals.

Offer strengths-based case management to help veterans maintain stable housing and employment.

Assist clients in accessing healthcare, mental health services, substance abuse treatment, and other necessary resources.

Utilize evidence-based practices such as Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First principles.

Monitor and adjust treatment plans as needed, ensuring clients receive the support necessary for long-term success.

Foster a non-judgmental, client-centered approach that respects the diverse backgrounds and experiences of those served.

What You Bring

We're looking for candidates who are empathetic, adaptable, and committed to helping others. Successful applicants will have:

A Master's degree in Social Work (MSW), Marriage & Family Therapy (MFT), Clinical Counseling, Psychology, or a related field.

Registration (or eligibility for registration) with the California Board of Behavioral Sciences (e.g., ACSW).

At least one year of experience in healthcare, case management, homeless services, or working with vulnerable populations.

Knowledge of evidence-based interventions, including Crisis Intervention, Motivational Interviewing, and Harm Reduction.

Strong computer proficiency with Microsoft Office and case management databases.

Professionalism, sound judgment, and the ability to work effectively in a fast-paced, team-oriented environment.

Requirements

Ability to work flexible hours, including evenings and weekends as needed.

Valid driver's license, reliable transportation, and proof of insurance.

CPR/First Aid certification (or willingness to obtain).

Ability to pass a background check, drug screening, and tuberculosis (TB) test.

Proof of full COVID-19 vaccination.

Work Expectations

This position requires on-site and field-based work in California.

Hybrid schedules may be available based on job responsibilities, but fully remote work is not an option.

Candidates must be legally authorized to work in the U.S. Sponsorship is not available.
